Medically reviewed by Dr. Troy Austin, MD — double board-certified plastic surgeon ◆ Updated July 2026

Transgender top surgery is gender-affirming chest surgery that helps your chest reflect your gender identity — either removing breast tissue to create a flatter, masculine chest (chest masculinization, or FTM top surgery), or using implants or fat transfer to build a fuller chest (transfeminine top surgery, or MTF breast augmentation). For many people it’s one of the most meaningful steps in their transition, and the goal is simple: a natural-looking result that lets you feel at home in your body. At Southeastern Aesthetic Surgery in Augusta, GA, board-certified plastic surgeon Dr. Troy Austin provides affirming, individualized care for transmasculine, transfeminine, and non-binary adults across the CSRA, greater Georgia, and South Carolina.

Your surgeon

An affirming surgeon, close to home.

For a lot of patients in east Georgia and the CSRA, top surgery has meant a long drive to Atlanta. Dr. Austin offers that same board-certified, gender-affirming surgical care right here in Augusta — a private, respectful practice where your goals lead the conversation. He is double board-certified by the American Board of Plastic Surgery and the American Board of Surgery, completed his fellowship at the Medical College of Georgia, and brings a plastic surgeon’s eye for a natural, well-proportioned chest.

Because he also performs liposuction, fat transfer, and breast augmentation, top surgery can be combined with contouring for a more complete result when that’s your goal — part of the full range of procedures at SEA.

Top surgery is about helping your body match who you already are. My job is to listen, be honest about what surgery can do, and give you a natural result you feel at home in. Dr. Troy Austin

Who is a candidate for top surgery?

Top surgery is for transgender and non-binary adults 18 and older in good general health who want their chest to better reflect their gender identity and have realistic expectations about surgery. Many surgeons, including our practice, follow the WPATH Standards of Care, which often means a supportive letter from a qualified mental health provider — something that also helps with insurance. You don’t necessarily have to be on hormone therapy to qualify. The best first step is a consultation, where Dr. Austin listens to your goals and walks you through what’s realistic for your body.

Techniques

Approaches, matched to your goals.

The right technique depends on your goals, your chest, and your skin. Here’s how the main options compare; Dr. Austin recommends the best fit at your consultation.

ApproachWho it’s often forWhat’s involved
Double incision + nipple graftMasculinizing medium to larger chestsRemoves breast tissue and contours a flat chest; the nipple-areola is resized and repositioned as a graft. Horizontal chest scars.
PeriareolarMasculinizing smaller chests, good skin elasticityIncisions around the areola; less scarring, and the nipple stays attached.
KeyholeMasculinizing very small chests, excellent elasticityA small incision at the base of the areola; minimal scarring, nipple stays attached.
Breast augmentationTransfeminine, building a fuller chestImplants and/or fat transfer create a natural, feminine breast contour tailored to your frame.

Not sure which fits? Your consultation is the place to find out — there’s no one-size-fits-all in top surgery.

Chest masculinization

FTM & transmasculine top surgery.

Masculinizing top surgery removes breast tissue and contours a flatter, more masculine chest — often the single most affirming step in a transmasculine person’s transition.

Shaping the chest

Beyond removing tissue, the goal is a chest that looks natural and proportioned — contouring the sides, defining the chest wall, and positioning the nipple-areola to look right for you. Liposuction is sometimes used to refine the sides.

Nipples & scars

Depending on your technique, the nipple-areola may be kept attached (periareolar, keyhole) or resized and placed as a graft (double incision). Double incision leaves horizontal scars that fade over time; we plan them thoughtfully, support your healing, and can offer scar revision later if you ever want it.

Non-binary goals

Not everyone wants a fully flat chest. If your goal is reduction or a different contour, we tailor the plan to what feels right for you.

Your result, your goals

From a fully flat chest to a subtler contour — the plan is built around you.

Transfeminine top surgery

MTF breast augmentation.

Transfeminine top surgery builds a fuller, feminine chest using implants, fat transfer, or both — tailored to your frame for a natural, balanced result.

Implants & fat transfer

Because the transfeminine chest often has a wider base and different starting anatomy, implant selection and placement matter. Dr. Austin chooses an implant — and, when helpful, adds fat transfer — to create a proportionate, natural-looking breast.

Working with hormones

If you’ve been on estrogen, you may already have some breast development; we plan your augmentation around where you are and what you want to achieve.

A natural look

The aim is a result that fits your body and your goals — the same board-certified breast augmentation expertise, applied with transfeminine anatomy in mind.

Tailored to your frame. Implant choice and placement are planned for a natural, feminine contour.

Investment

How much does top surgery cost in Augusta?

Cost depends on your procedure and technique. Top surgery at SEA is self-pay — we don’t bill insurance directly, but after surgery we provide the codes you need to file for reimbursement, and financing can cover the cost up front. We give you a clear, personal quote at your consultation.

Chest masculinization (FTM), typical self-pay $9,000 – $12,000
Transfeminine breast augmentation (MTF), typical self-pay $6,500 – $10,000
Possible insurance reimbursement Filed after surgery
Consultation with Dr. Austin $100

Self-pay ranges are typical estimates and vary with technique, anatomy, and whether procedures are combined. We don’t bill insurance directly, but we provide the codes you need to file for reimbursement after surgery. Financing is available through Cherry, CareCredit, PatientFi, and Alle Financing.

Insurance & letters

Coverage, WPATH & getting ready.

Southeastern Aesthetic Surgery does not accept insurance for top surgery — but we make filing for reimbursement straightforward, and financing helps cover the cost up front.

How insurance works here

We do not bill insurance directly. After your surgery is completed, we provide the necessary codes so you can file a claim with your insurance company for possible reimbursement. To offset the initial cost before any reimbursement comes back, we offer third-party financing. You’re also welcome to contact your insurance company after your consultation to learn how the process will work with your specific plan.

WPATH letters

Following the WPATH Standards of Care, a supportive letter (or letters) from a qualified mental health provider is often part of the process — both for readiness and for insurance. We’ll tell you clearly what’s needed for your situation.

Getting ready

From the first consultation, our team helps you navigate documentation, timing, and financing so the path to surgery is as clear as possible.

Good to know

Top surgery Questions.

What is transgender top surgery?

Top surgery is gender-affirming chest surgery. For transmasculine patients it removes breast tissue to create a flatter, masculine chest (chest masculinization); for transfeminine patients it uses implants and/or fat transfer to build a fuller chest. The goal is a natural result that helps your chest reflect your gender identity.

FTM (female-to-male) or masculinizing top surgery removes breast tissue and contours a flat chest. MTF (male-to-female) or transfeminine top surgery is a breast augmentation that builds a fuller chest with implants or fat transfer. We also tailor plans for non-binary patients whose goals fall outside either.

The main options are double incision with a free nipple graft (best for medium to larger chests), periareolar (smaller chests with good skin elasticity), and keyhole (very small chests with excellent elasticity). The right one depends on your chest size, skin, and goals.

Self-pay chest masculinization typically runs about $9,000 to $12,000, and transfeminine breast augmentation about $6,500 to $10,000, depending on technique and anatomy. We don’t bill insurance directly, but we provide the codes so you can file for reimbursement after surgery. You’ll get an exact quote at your consultation.

Not necessarily. Hormone therapy isn’t always required for top surgery, though it can affect your starting anatomy, especially for transfeminine augmentation. We’ll plan around where you are in your journey.

You’ll wear a compression garment or binder, and masculinizing surgery often includes temporary drains. Most people take one to two weeks off from work and avoid heavy lifting and strenuous activity for several weeks. We guide you through each stage of healing.

Some techniques (double incision) leave chest scars that fade over time; others (keyhole, periareolar) leave less. Nipple sensation can change, especially with a free nipple graft. We’ll go over what to expect for your specific technique honestly before surgery.

No. Our practice focuses on top surgery and related chest and body procedures. We do not perform bottom (genital) surgery, and we do not have a referral provider to send patients to for it.
